Attached is this year’s revised Federal Contractor Veterans’ Employment Report 2019 VETS-4212 form. The form is already populated with The University of North Carolina’s company information. In addition to reporting information on the total number of employees who are protected veterans, the following fields need to be completed:
- Twelve Month Period Ending
- Name and Address of Hiring Location
- DUNS # (nine-digit business identifier for all procurement-related activities)
- Employer ID (IRS Tax No.)
Please complete and return this form to me on or before September 20, 2019. DO NOT send your completed form directly to DOL-NCC; the UNC System Office submits all of the 17 constituent institution reports at one time as “Prime Contractor – Multiple Establishment-Hiring Location.”
