Per the state’s Bereavement Leave Policy, universities are charged with collecting and reporting the usage of bereavement leave to OSHR each year. The bereavement leave report should include leave used during the period of July 1, 2025 – June 30, 2026, the report is due to OSHR by close of business on August 31, 2026.

Please do not change the format of the template (data must be consolidated from all agencies and universities).
